If you are a local subsidiary of a national or international organization, or a university or hospital, contact grants management before continuing with registration: (312) 899-5012. If you are a New User from an organization using an IRS registered charitable organization as fiscal sponsor, please follow these steps:

1. Enter your organization's information in the fields below.
2. Click Search IRS Database.
3. Use the pop-up window to search for your fiscal sponsor by name or EIN number.
4. If multiple entries appear, add the city name and click Search IRS Database to narrow the results.
5. Select your fiscal sponsor from the search results. The search window will disappear.
6. The form below will automatically populate with the fiscal sponsor's contact information directly from the IRS database.
7. Complete the Contact Information fields with your own information, then click Submit. Fields marked with "*" are mandatory).
